Employee Benefits: Insurance Plans
 
Health Insurance
 
  The Health Insurance program is administered through the Office of Group Benefits; which is a self-insured and self-funded comprehensive medical benefits plan provided by the State of Louisiana.

New employees must enroll within the first 30 days of employment. Employees and dependents over age 19 who apply for coverage are subject to a pre-existing condition limitation. Under the pre-existing limitation, no benefits are payable during the first 12 months of coverage in connection with any disease, illness, accident, or injury diagnosed or treated during the 6 months prior to the enrollment date. Pregnancy is not considered a pre-existing condition.

You may be exempt from the all or part of the pre-existing condition limitation if you were continuously covered under another health plan within 63 days prior to the effective date of coverage in this program.

You must complete an enrollment form within 30 days after acquiring a new dependent by: birth, adoption, marriage, etc.) If you do not do so, your dependent may be subject to the pre-existing condition limitation.

You may still enroll in a plan after the 30 days, however, you will be considered a late applicant and must provide proof of insurability. If you choose not to enroll you must complete a waiver of coverage. The employee and the university share the cost of the premium.

Effective date of Coverage:
  • New Hires and Transfers:
    If employment begins on the 1st day of the month, coverage is effective the 1st day of the following month. If employment begins on the second day of the month or after, coverage is effective the 1st day of the second month following employment.

  • Late Applicants:
    The effective date of coverage for applicants whose forms are received prior to the 14th of the month will be the first day of the month following the date of receipt of all required forms. The effective date of coverage for applicants whose forms are received on or after the 14th of the month will be the first day of the second month following receipt.
Below is a list of the various plans:
  • Office of Group Benefits – PPO (Preferred Provider Organization)
  • United Health Care – CDHP-HSA Plan (Consumer Driven Health Plan)
  • Blue Cross Blue Shield– HMO (Health Maintenance Organization)

Life Insurance
 
  Prudential Insurance Company of America underwrites the life insurance coverage offered through Office of Group Benefits. This is a life insurance benefit based on your annual salary to a maximum of $50,000.00. There are also provisions for benefits for your dependents, and there is an additional provision for accidental death and dismemberment coverage for employees. The employee and the University share the cost of the premium. The employee pays the entire premium for the dependent life. For more information you can visit the website: www.groupbenefits.org
 
Disability Insurance
 
  Long Term Disability Benefits are provided by MetLife. After a three month qualifying disability period, benefits are paid at 60% of the employee’s basic monthly salary to a maximum monthly benefit of $4,000.00. You pay the entire premium.
 
Dental Insurance
 
  Dental Insurance is provided by Crescent Dental and underwritten by Companion Life Insurance Company. This is a voluntary incentive plan that offers coverage for four types of dental services (preventative, basic, major and orthodontic services) with a freedom of choice of dentist. You pay the entire premium.

Although you may go to any dentist, Crescent Dental contracts with DenteMax, a Preferred Provider Network of independent dentists. The dentists who are part of this network have agreed to accept a set fee schedule when receiving payment for dental services. When you choose a DenteMax dentist, you can save out-of-pocket expenses. *Note: If you do not enroll within 30 days of employment, you will not be eligible to join until open enrollment; which is from October 1 through November 30 of every year. The effective date of coverage would be January 1st of the following year.
 
Vision Insurance
 
  Vision Insurance is provided by Crescent Dental Plans, with premiums collected through payroll deduction. You pay the entire premium. Active employees who are eligible for benefits can enroll in the Crescent Vision Plan even if not enrolled in the Crescent Dental Plan. This is a voluntary plan that offers coverage for eye exams with dilation (as needed), contact lens fit and follow-up, frames, standard plastic lenses, lens options, and contact lenses (material only).

Although you may go to any provider, Crescent Dental contracts with EyeMed Vision Care. By selecting a provider from the EyeMed Access Network you will receive the maximum benefits. For a list of EyeMed Providers, go to www.enrollwitheyemed.com; choose the Access Network, enter your zip code.

*Note: If you do not enroll within 30 days of employment, you will not be eligible to join until open enrollment; which is from October 1 through November 30 of every year. The effective date of coverage would be January 1st of the following year.
