The University invites all employees to self-identify their status as an individual with a disability (IWD) and disabled veteran. 

WHY AM I BEING ASKED TO SELF-INDENTIFY?
Because the University does business with the government, we must reach out to hire and provide equal opportunity to qualified people with disabilities.  To help us measure how well we are doing, we are asking you to tell us if you have a disability,  if you've ever had a disability, or if you are a disabled veteran. 

The submission of this information is voluntary, but we hope that you will choose to self-identify as such.  Refusal to disclose this information will not subject you to any adverse treatment. Employees may voluntarily provide this information electronically by logging on to ULink , selecting the Employee tab, then clicking on “Banner Self Service”  under the “Banner Links” tab.   Employees should then click on the “Personal Information”  tab and complete the  “Veterans Classifications” and “Disability Status”  sections.  Paper self-identify forms are also available.  These forms can be obtained from The Office of Human Resources (104 University Circle, Martin Hall, Rm 170.)

Because a person may become disabled at any time, you may update your status at any time in the future as well. 

The University will extend this invitation to self-identify to employees every five years hereafter.
